To start this trail, pass the entrance station for Mathews Arm Campground and park in the lot. Head back towards the entrance station and head left this time on Knob Mountain Rd toward the trailer sewage disposal facility and look for the yellow-blazed Knob Mtn Spur, which marks the beginning of the trail at the end of the loop.

This yellow-blazed trail was originally a fire road that headed to the base of the summit of Knob Mountain. Being in a wilderness area, the road has been closed to vehicles and reclassified as a trail. It heads along the east side of the mountain, mostly downhill. The trail heads out for a few miles along the crest of the ridge. At a low point between two knobs, in an old clearing, look to the left for the concrete marker post that shows the blue-blazed Knob Mtn Cutoff Trail.

Continue ahead, going uphill toward the high point of the trail at around 2,850'. From here, the trail descends along the ridge, with a few sections of switchbacks and some steep stretches. One final large switchback marks the end of the trail as it comes in parallel to Jeremys Run and ends at the junction with Jeremys Run Trail and Neighbor Mountain Trail.
